Requirements for various services

A. LICENSES / PERMISSION ISSUED BY THE VILLAGE PANCHAYAT

1) Grant of License for Construction / Reconstruction of any Structure

The following are the documents required

  • Once received by the Panchayat the same is placed before the Panchayat meeting.
  • Site inspection conducted, if decided by the Panchayat.
  • If found in order and there are no differences of opinion between the V.P. Secretary, Sarpanch, and V.P. member, (i.e. the inspection team) construction license is issued, else placed for the Panchayat meeting again.
  • Minimum time taken to issue is within 15 days on approval after placing the file for Panchayat meeting.

2) Permission for Repairs of House / Structure

Requirements are as follows

  • Application in the prescribed proforma.
  • Ownership Documents / Rough Sketch.
  • House Tax Receipt
  • Placed for the Panchayat meeting and forwarded to B.D.O
  • Placed for the Panchayat meeting once approval is received from B.D.O. Site inspection is conducted, if decided by the Panchayat.
  • If found in order and there are no differences of opinion between the V.P. Secretary, Sarpanch, and V.P. member, (i.e. the inspection team) permission is issued. Else placed for the Panchayat meeting again.
  • Permission issued within 10 days from the date of approval of meeting.

3) License for Hotels / Shops/ Restaurant/ Eating House/ Coffee House/ Sweet Mart Shop/ Bakery/ Boarding etc.

Requirements are as follows

  • Application from the applicant mentioning all details.
  • Swearing on an affidavit with details of the NOC sought
  • House Tax Receipt / Ownership Documents/lease agreements, else an NOC from the owner
  • Swearing on Stamp Paper for safe disposal of garbage
  • Placed for the Panchayat meeting
  • Site inspection conducted, if decided by the Panchayat.
  • If found in order and there are no differences of opinion between the V.P. Secretary, Sarpanch, and V.P. member, (i.e. the inspection team) License as requested is issued, else placed for the Panchayat meeting again.
  • Minimum time taken to issue is within 10 days on approval after placing it for the Panchayat meeting.

4) License for using any place for Trade, Business or Industry

The requirements are as follows.

  • Application from the applicant to with details of place where the Trade / Business or Industry is to be established.
  • House Tax receipt / Ownership Document or NOC from the owner.
  • Swearing on Stamp Paper for safe disposal of Garbage
  • NOC from fire dept.
  • NOC from Health Dept.
  • NOC from Food and Drugs Dept.
  • Any other NOC’s as the case may be

B. CERTIFICATES ISSUED BY THE VILLAGE PANCHAYAT

1) Income Certificate

Requirements are as follows:-

  • Application from the applicant
  • Ward Member signature on the application
  • Self Declaration with photo affixed of the applicant
  • ID proof (House Tax Receipt / Ration Card / Epic Card / Adhar Card)
  • Samaj Certificate (If applied for Caste Certificate from Dy. Collector)
  • Marriage Certificate
  • Any other document concerning the purpose of the certificate

2) Birth/Death Certificate

Requirements are as follows:

  • Application from the applicant with details of Birth / Death
  • Minimum time taken to issue is within 2 days.

3) Occupancy Certificate

Requirements are as follows:-

  • If found in order and there are no differences of opinion between the V.P. Secretary, Sarpanch, and V.P. member, (i.e. the inspection team) occupancy is issued, else placed again for the Panchayat meeting.
  • Minimum time taken to issue is within 10 days on approval after placing it for the Panchayat meeting.

4) No Dues Certificate

Requirements are as follows:-

  • Application from the applicant
  • House Tax paid receipt
  • The minimum time taken is within 7 days after verification of the application in the Panchayat records.

5) Non Availability of Birth or Death Certificate

Requirements are as follows:-

  • Application from the applicant
  • Swearing on an affidavit from the applicant giving details of Birth or Death
  • True copy of the documentary evidence for stating the Birth or Death event in V.P. Navelim Jurisdiction.
  • Minimum time taken to issue is within 7 days after verification of the application in the Panchayat records.

6) Residence Certificate

Requirements are as follows:-

  • Application from the applicant mentioning the details as required to be filled in on FORM III stating the purpose for which it is required and where to be produced.
  • Ward Member signature on the application
  • Birth Certificate
  • ID proof (Ration Card / Epic Card)
  • Minimum time taken to issue is within 10 days after verification of the application in the Panchayat records.

7) Character Certificate

Requirements are as follows:-

  • Application from the applicant mentioning the details of the purpose for which it is required and where to be produced.
  • Ward Member signature on the application
  • Birth Certificate
  • ID proof (Ration Card / Epic Card)
  • Minimum time taken to issue is within 7 days after verification of the application and enclosures.

8) Dependency Certificate

Requirements are as follows:-

  • Application from the applicant mentioning the name of the dependant and the department where the Certificate is to be produced
  • Ward Member signature on the application
  • ID proof (Adhaar card / Epic Card)
  • Minimum time taken to issue is within 7 days after verification of the application and enclosures.

9) Poverty Certificate

Requirements are as follows:-

  • Application from the applicant mentioning the department where the Certificate is to be produced
  • Ward Member signature on the application
  • ID proof (Adhaar Card / Epic Card)
  • Minimum time taken to issue is within 7 days after verification of the application and enclosures.

10) Divergence Certificate

Requirements are as follows:-

  • Application from the applicant mentioning the purpose of the certificate and department where the Certificate is to be produced
  • Ward Member signature on the application
  • ID proof (Adhaar Card / Epic Card)
  • Documents pertaining to the divergence of names sought.
  • Minimum time taken to issue is within 7 days after verification of the application and enclosures.

C. NO OBJECTION CERTIFICATES (NOC’s) ISSUED BY THE VILLAGE PANCHAYAT

1) NOC for Water Connection

Requirements are as follows:-

  • Application mentioning the H. No., name of the Owner and purpose
  • Copy of House Tax Receipt
  • NOC from the Owner if the Ownership if House Tax Records differ.
  • Placed for the Panchayat meeting
  • Site inspection conducted, if decided by the Panchayat.
  • If found in order and there are no differences of opinion between the V.P. Secretary, Sarpanch, and V.P. member, (i.e. the inspection team) NOC for Water Connection is issued, else placed for the Panchayat meeting again.
  • Minimum time taken to issue is within 4 days on approval after placing it for the Panchayat body meeting.

2) NOC for Electricity Connection

Requirements are as follows:

  • Application mentioning the H. No., name of the Owner and purpose
  • NOC from the Owner if the Ownership if House Tax Records differs.
  • Placed for the Panchayat meeting
  • Site inspection conducted, if decided by the Panchayat.
  • If found in order and there are no differences of opinion between the V.P. Secretary, Sarpanch, and V.P. member, (i.e. the inspection team) NOC for Electricity Connection is issued, else placed for the Panchayat meeting again.
  • Minimum time taken to issue is within 4 days on approval after placing it for the Panchayat body meeting.

3) NOC for Road Cutting

Requirements are as follows:-

  • Application from the applicant to cut the road with exact address
  • The purpose for which the road cutting is required and place where the NOC is to be submitted
    NOC from the Owner if necessary.
  • Minimum time taken is within 4 days on approval after placing it for the Panchayat meeting.
  • Placed for the Panchayat meeting
  • Site inspection conducted, if decided by the Panchayat.
  • If found in order and there are no differences of opinion between the V.P. Secretary, Sarpanch, and V.P. member, (i.e. the inspection team) NOC for Road Cutting is issued, else placed for the Panchayat meeting again.
  • Minimum time taken to issue is within 4 days on approval after placing it for the Panchayat body meeting.

4) NOC for Business

Requirements are as follows:-

  • Application from the applicant to with details of place where the General Store business is to be started.
  • House Tax receipt / Ownership Document or NOC from the owner.
  • Self Declaration Paper for safe disposal of Garbage
  • NOC from fire dept.
  • NOC from Health Dept.
  • NOC from Food and Drugs Dept.
  • Placed for the Panchayat meeting
  • Site inspection conducted, if decided by the Panchayat.
  • If found in order and there are no differences of opinion between the V.P. Secretary, Sarpanch, and V.P. member, (i.e. the inspection team) NOC for Running General Store is issued, else placed again for the Panchayat meeting.
  • Minimum time taken to issue is within 10 days on approval after placing it for the Panchayat body meeting.

5) Transfer of House Tax Records

Requirements are as follows:-

  • Application from the concerned.
  • Deed of Sale & Agreement of Sale
  • House Tax receipt / Ownership Document or NOC from the owner.
  • Self Declaration for safe disposal of Garbage
  • If the Owner is deceased, Copy of Death Certificate to be attached
  • Self Declaration stating the NOC from other legal heirs. Copy of Death Certificate to be attached incase any heir is deceased.
  • Placed for the Panchayat meeting
  • If found in order and there are no differences of opinion between the Panchayat than the same is transferred in the name of the applicant.
  • Minimum time taken to transfer is limited to payment of transfer fees of Rs. 1000/- by the applicant after the same is approved for the Panchayat body meeting.

D. OTHER SERVICES

1) Correction in Births and Deaths Records

Requirements are as follows:-

  • Application from the applicant mentioning the corrections to be done.
  • Swearing on Stamp Paper mentioning the erroneous names and the correct names
  • Relevant Proof / certificates to support the correction
  • Minimum time taken is within 15 days after scrutinizing the application and enclosures.
  • Work load per month is 1-2 on an average.
  • The facility is available and in operation.
  • This service cannot be applied online.

2) Issue of Certified copies of Resolutions

Requirements are as follows:-

  • Application from the applicant mentioning the Resolution number and date of the resolution taken.
  • Minimum time taken is within 7 days after scrutinizing the application and enclosures.

3) Issue of Information under RTI Act, 2005

Requirements are as follows:-

  • Application from the applicant mentioning the details of information sought to be furnished under RTI, 2005.
  • Court Fee of Rs. 10 on the application.
  • Maximum time taken is within 30 days.

NOTE:
Application Fees of Rs. 5/- will be applicable for Application for Income Certificate, Births / Deaths Certificate & FORM III at the time of submitting the same to this office
